Does Insurance Cover a Doula in the Bay Area? 2026 Guide

If you are pregnant or recently gave birth in the San Francisco Bay Area, you may be wondering whether your health insurance covers birth or postpartum doula services.

The answer depends on your health plan. Medi-Cal provides doula services as a covered statewide benefit, and several Bay Area managed care plans provide that benefit to their eligible members. Certain Kaiser Permanente California plans also cover services provided by contracted doulas.

I provide bilingual birth and postpartum doula support for eligible families in Contra Costa County, Alameda County, San Francisco, and select surrounding Bay Area communities. Which Bay Area insurance plans may cover a doula?

Medi-Cal


Doula services became a covered Medi-Cal benefit in California on January 1, 2023. The benefit is available through both fee-for-service Medi-Cal and Medi-Cal managed care plans.


Covered support may include emotional, physical, educational, and nonmedical assistance during pregnancy, labor, birth, and the postpartum period. Medi-Cal doula services may also be available following miscarriage, stillbirth, or abortion.


Eligible members generally have access to:

  • One initial doula visit
  • Up to eight additional prenatal or postpartum visits
  • Support during labor and delivery
  • Up to two extended three-hour postpartum visits
  • Additional postpartum visits may be available when the appropriate requirements are met.

Contra Costa Health Plan


Contra Costa Health Plan lists doula care as a covered service for eligible members. Its published benefit includes an initial visit, additional prenatal or postpartum appointments, labor and delivery support, and extended postpartum visits.


Families looking for a Contra Costa County doula should still confirm current membership, eligibility, and whether their selected doula can bill the plan.


San Francisco Health Plan


San Francisco Health Plan provides doula services to eligible Medi-Cal members before, during, and after pregnancy. Its member information states that doula support may be received in settings such as the home, hospital, office, or birth center.


If you have SFHP and are looking for a birth doula in San Francisco, your coverage and the doula’s provider status should be checked before scheduling services.


Alameda Alliance for Health


Alameda Alliance for Health offers doula services as a covered benefit for eligible Medi-Cal members. The plan describes doulas as birth workers who provide health education, advocacy, and physical, emotional, and nonmedical support throughout pregnancy, birth, and postpartum recovery.


Families searching for a doula in Alameda County should confirm that the doula is appropriately enrolled or contracted with the plan.


Partnership HealthPlan of California


Partnership HealthPlan covers doula services for eligible members during pregnancy, labor, birth, and up to one year after the end of pregnancy. Partnership describes doula care as including health education, advocacy, guidance, and nonmedical support.


Partnership serves multiple Northern California counties, so availability will also depend on the doula’s service area and travel distance.


Kaiser Permanente


Kaiser Permanente states that services from contracted doulas are covered at no extra cost for California members enrolled in specific qualifying plans. Members should review their Evidence of Coverage or contact Member Services to confirm whether their individual plan includes the benefit.


Having Kaiser insurance does not automatically mean that every doula can bill Kaiser. The doula generally needs to meet Kaiser’s contracting or network requirements for the member’s particular plan.

What does insurance-covered doula care include?

Follow these steps before beginning services:


1. Identify your complete health plan


“Medi-Cal” alone may not be enough information. Look at your insurance card to determine whether you have:

  • Contra Costa Health Plan
  • San Francisco Health Plan
  • Alameda Alliance for Health
  • Partnership HealthPlan of California
  • Kaiser Permanente
  • Fee-for-service Medi-CalAnother managed care or employer-sponsored plan

2. Check whether the doula works with your plan


A doula may participate with some plans but not others. Ask whether the doula is enrolled, credentialed, contracted, or otherwise permitted to bill your specific health plan.


3. Confirm the service area and estimated due date


Insurance coverage does not automatically guarantee that a particular doula is available. Your location, expected delivery date, hospital, and travel distance must also be considered.


4. Complete the coverage-verification process


You may need to provide your:

  • Health plan name
  • Member identification number
  • Date of birth
  • Estimated due date
  • City of residence
  • Delivery hospital, if known
  • Sensitive information should only be submitted through the provider’s designated secure process.

5. Wait for confirmation before scheduling covered visits


Coverage should be verified before beginning services. Verification helps confirm your eligibility, the doula’s ability to bill your plan, and any authorization or documentation requirements.
